The tarsal tunnel syndrome results from the entrapment of the tibial nerve behind the medial malleolus. It is less frequent than the carpal tunnel syndrome.

The endoscopic carpal tunnel release technique can be transposed to the foot. The interest of this technique is its simplicity, the aesthetic benefit of reduced scar formation, and the absence of shoe problems. There are four basic requirements: feasibility, safety, easy reproducibility from operator to operator, and reasonable cost. The instrumentation for endoscopic carpal tunnel release is reusable and therefore the operation is cost-effective.

The carpal tunnel syndrome results from impairment of the median nerve in the carpal groove transformed into an inextensible fibro-osseous tunnel by the transverse carpal ligament (TCL).

There is a clear interest in releasing the median nerve under endoscopy according to Agee's (USA) single-portal technique. The operative procedure is facilitated by use of the ORTHOMED instrumentation, suited to all types of optical systems, providing optimum endoscopic visualization with no condensation, and one-pass sectioning of the TCL by a retrograde blade from distal to proximal.
